About The Role:
As a Palliative Care Nurse Practitioner, you will play a crucial role in assessing, planning, implementing, and evaluating care for patients with serious, life-limiting illnesses. Your responsibilities will include:
- Providing comprehensive symptom management, pain relief, and psychosocial support. support to patients and their families.
- Conducting thorough assessments and developing individualised care plans to improve the quality of life for patients.
- Offering education on advanced illness, treatment options, and end-of-life care tailored to each patient’s needs.
- Collaborating with an interdisciplinary team to ensure holistic, patient-centred care.
- Maintaining accurate documentation and advocating for compassionate, dignified care at all stages of illness.
What We’re Looking For:
- Qualifications: Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) AND endorsed to practice as a Nurse Practitioner by the Nursing and Midwifery Board of Australia ( mandatory)
- Experience: Proven experience as a Nurse Practitioner, with exceptional clinical skills and expertise. Experience in the aged care sector is highly desirable, with a deep understanding of the complexities of aging, polypharmacy, Alzheimer’s disease, other dementias, and related conditions.
