About the Role:
As a Registered Nurse (RN), you will be dedicated to providing exceptional care and have the skills to work collaboratively to achieve the best possible outcomes for patients. You'll put your expert relationship-building skills into practice and always demonstrate the core nursing values of patient-centred care, trust and respect in every interaction with patients, carers and colleagues. You'll be accountable for establishing and evaluating the patient's plan of care, in partnership with the patient, their carers and other members of the health care team. You will be supported in your role as a nurse by all other nursing positions.
Key Responsibilities:
Patient Care
- Systemically assessing the patient and identifying the patient's nursing needs
- Applying knowledge and skills to develop and document a plan of care that reflects the identified needs of the patient
- Implementing the plan of care by providing direct care to patients
- Evaluating and validating the effectiveness of nursing strategies towards meeting expected patient outcomes
- Involving the patient and carers in the plan of care
- Recognising changes in the patient's condition, taking necessary action and documenting variation in the plan of care
Safety and Development
- Accepting accountability for your own actions and seeking guidance when a situation esc
- Coordinating a safe, effective and efficient discharge or transfer of care from the health service that reflects the needs of the patient and carers and ensures comprehensive communication of health information for the transition of care
- Developing, implementing and evaluating teach plans for patients and carers that meet learning needs and facilitates informed decision-making
- Developing and maintaining collaborative relationships with all disciplines
- Supporting and participating in evidence-based programs to evaluate and improve the quality of nursing care and patient outcomes
- Performing other job related duties as directed consistent with the Award Classification
