What You'll Do

• Conduct home assessments and create individualised care plans
• Deliver high-quality, person-centred clinical care in participants' homes
• Build trusted relationships with participants, families, and health teams
• Lead quality care initiatives and ensure compliance with aged care standards
• Support continuous improvement and accreditation projects
• Guide and mentor less experienced staff where appropriate

What We're Looking For

• Registered Nurse (RN) with current AHPRA registration - minimum 3 years' clinical experience
• Previous Clinical Care Partner experience is a plus, but not required
• Strong knowledge of aged care legislation and in-home care programs
• Excellent communication, interpersonal, and stakeholder management skills
• NSW Driver's Licence and confidence using IT systems
• Passion for providing participant-centred, high-quality care
