In this autonomous, community-based role, you will deliver high-quality clinical assessments, tailored exercise programs, and rehabilitation interventions directly to clients in their homes. You’ll work collaboratively with families and multidisciplinary teams to ensure holistic, person-centred care.

Key Responsibilities:

• Conduct comprehensive, needs-based clinical assessments
• Develop and implement personalised exercise and rehabilitation programs
• Deliver one-on-one and small group sessions focused on strength, balance, mobility, and functional independence
• Monitor outcomes and maintain accurate clinical documentation
• Collaborate with families and the broader care team
• Contribute to quality improvement, audits, and reporting requirements
• Ensure services meet legislative and organisational standards

About You:

• Tertiary qualification in Exercise Physiology
• Current ESSA registration
• Minimum 2 years’ clinical experience
• Experience working with older adults or in community/rehab settings highly regarded
• Strong assessment, rehabilitation, and equipment prescription skills
• Confident working autonomously and managing your own schedule
• Excellent communication and relationship-building skills
• Open to relocating to Canberra
