About the Role

As a Social Worker, you will play a key role in empowering individuals living with psychosocial disabilities to achieve their goals and improve their quality of life. Working within a multidisciplinary allied health team, you will provide both coordination and direct support to clients and their families.

Your responsibilities will include:

• Delivering psychosocial (mental health) support and assessments
• Preparing plan review reports and biopsychosocial/family assessments
• Developing and implementing individual and budget plans
• Collaborating with allied health professionals to coordinate support services
• Supporting individuals and families experiencing social disadvantage
• Providing coordination of supports or specialised support coordination

Key Requirements:

• Bachelor's Degree in Social Work or related in Australia
• AASW Registration
• Strong verbal, written, and interpersonal communication skills
• Excellent time management and organisational skills
• Active listening skills and empathy
• Valid Working with Children Check, NDIS Worker Screening, Police Check (within 6 months), Driver's Licence, car insurance, and a reliable vehicle
• Proficiency in Microsoft Office (Word, Excel, Outlook)
• Recent graduates and candidates with no prior experience are welcome to apply

Desirable:

• Experience with clients with dual diagnoses, forensic disability, or forensic mental health
• Knowledge and experience working within the NDIS framework
• Experience working with Culturally and Linguistically Diverse (CALD) communities
• Willingness to travel locally as required
