We're partnering with a well-established aged care provider to recruit a Clinical Care Coordinator for a coastal facility in this highly sought-after Western Australian location.
The Opportunity
Full-time, Monday to Friday
Supportive leadership structure, reporting into an experienced Clinical Manager
Work alongside another CCC in a collaborative environment
Potential Relocation support + sponsorship available for candidates within Australia
The Role
You'll play a key role in leading clinical care delivery and supporting a high-performing team, with a strong focus on quality, compliance, and resident outcomes.
Oversee clinical care coordination and delivery
Support and mentor nursing and care teams
Ensure compliance with Aged Care Quality Standards
Manage clinical documentation, care planning & AN-ACC processes
Act as a key point of contact for families and stakeholders
Contribute to audits, quality improvement, and clinical governance
About You
AHPRA Registered Nurse
2+ years' experience in a clinical leadership role
Strong understanding of aged care standards & compliance
Confident leading, mentoring, and supporting teams
Experience with audits, accreditation & continuous improvement
Strong communication and stakeholder engagement skills
