Start your nursing career with Grampians Health Aged Care supported with a Transition to Practice Program. The Transition to Practice Program provides structured training, mentoring and support during your first 12 month of employment.
You will work alongside experienced Registered Nurses and Clinical Support Nurses to develop and consolidate your skills while delivering high quality, person-centred care to our aged care residents.
Key Responsibilities
Deliver nursing care in consultation with Registered Nurses
Monitor resident wellbeing and report clinical changes
Support resident assessments and care planning
Maintain accurate clinical documentation
Assist with medication and treatment delivery
Contribute to quality improvement and safe care
Skills & Experience
New graduate with less than 12 months of experience as an Enrolled Nurse post AHPRA registration.
Current NDIS Worker Screening Check (or willingness to obtain).
A commitment to providing exceptional care and a passion for working in Aged Care.
Visa Sponsorship Support (if required)
407 Training Visa sponsorship available for eligible candidates with a potential pathway and employer support to permanent residency via TSS482 and ENS186.
Relocation support up to $12,000 for eligible applicants.
Accommodation support up to 6 weeks (subject to availability).
