We are seeking a dedicated Occupational Therapist to join our growing Home Care team in Toowoomba on a full-time permanent basis. This is an exciting opportunity to work within a supportive multidisciplinary team, delivering person-centred care to older Australians and community clients.

About the Role
As a Home Care Occupational Therapist, you will provide comprehensive assessments, therapy, equipment prescription, home modifications, and rehabilitation services that enable clients to live safely and independently in their own homes.
Key Responsibilities
Conduct occupational therapy assessments in clients' homes.
Develop and implement personalised intervention and rehabilitation plans.
Recommend and prescribe assistive technology and equipment.
Complete home safety assessments and recommend home modifications.
Support clients with falls prevention, mobility, daily living activities, and independence.
Work collaboratively with GPs, allied health professionals, families, and care teams.
Maintain accurate clinical documentation and reports.
Deliver high-quality, person-centred care in accordance with professional standards.
About You
You will have:
A recognised qualification in Occupational Therapy.
Current registration with AHPRA
Excellent communication and interpersonal skills.
Ability to work independently while managing a mobile caseload.
Current Australian Driver Licence and reliable vehicle
National Police Check.
NDIS Worker Screening Check (or willingness to obtain).
Current First Aid and CPR certification (or willingness to obtain).
Previous experience in community health, aged care, rehabilitation, or home care is desirable but not essential.
